No Show Fees

Patients who cancel their appointments less than 24hrs in advance or do not show up for their appointment will be charged $20 ($50 for Saturday or after-hours appointments).  Re-scheduling may require an additional deposit (refundable at the time of actual visit)

No professional services are provided on or through this website. Telemedicine services may be provided, but only through a separate electronic health record (EHR) platform; there are no fees or services related to this website. Complaints and/or feedback regarding this website and the quality of information and services can be sent to info@excelsis.life or the State Board.

 

After-hours & Response Times: Providers

Excelsis will diligently safegaurd against any potential and forseeable problems or complications related to your medical care. Since side effects or treamtents and surgical procedures, such as bleeding or infection, most commonly occur withing the first 10-14 day, surgeries will be scheduled only when we have the ability to respond quickly during that time frame. Providers of this practice will always be available, directly or indirectly, to Medical Facilities 24 hours a day and we will also use a covering or backup neurosurgeon.  Therefore, neurosurgery providers outside of this Practice may be involved in your care, especially for unexpected, unforeseeable, and/or emergency situations.

  

After-hours & Response Times: Patients

Excelsis Response times for non-medical persons (patients/families) will be triaged, meaning that they will vary based on the the degree of medical urgency. Please note that telephonic and/or online communications may not be adequate for assessment, so therefore patients should always consider face-to-face and hands-on assessment by an ER physician.  For non-urgent topics, our  approximate response times will typically be: less than 24 hours via telephone/voicemail, 12 hours via Portal message, and 72 hours via email or fax.  We sometimes return calls in the evening and on weekends;  please tell us when we to call you during those times. 

 

Doctor-Patient Relationship

Trust and mutually respect are paramount to a sucessful doctor-patient relationship. No doctor-patient relationship exists until actual care is provided and there are circumstances where this relationship can be terminated. Patient non-compliance and disruptive behavior can create justifications for unilateral severance of the relationship. Notice will be given with timing based on the on the status of any medical problems and we will offer to facilitate transfer of care when indicated. Failure to respond to office commmunications can be used as evidence of self-termination. Likewise, we would appreciate that you inform us if you intend to seek care elsewhere; we can also provide your medical records.
